Adult Day Services

Adult Day Services Medical Adult Day Services or Adult Health Services in NYS

  •  Medically-supervised services for individuals with physical or mental impairment (examples: children, people with dementia, or AIDS patients)
  • Services include: Nursing, transportation, leisure activities, physical therapy, speech pathology, nutrition assessment, occupational therapy, medical social services, psychosocial assessment, rehabilitation and socialization, nursing evaluation and treatment, coordination of referrals for outpatient health, and dental services. Respite.

Social Adult Day Programs

Care for frail and disabled older adults in a supervised group setting within the community. Services include socialization activities, supervision and monitoring, and nutrition. Programs also provide personal care that includes hands on assistance with mobility, eating, and toileting (including care of incontinence).

Aging in Place

The U.S. Centers for Disease Control and Prevention defines aging in place as “the ability to live in one’s own home and community safely, independently, and comfortably, regardless of age, income, or ability level.”

 

In gerontology, an Elder Village or Senior Village (occasionally virtual village, and usually shortened to Village) is an organization, usually staffed by volunteers (often with a small paid staff), that provides services to the elderly in order to allow them to remain in their homes as they age. Villages are a part of the aging in place movement, and are found in the United States, Canada, Australia, and the Netherlands.

 

Most Villages have members, to whom they provide services upon request. Services offered typically include transportation, light home maintenance and repair, and social activities. Most Villages do not provide medical services or involved home maintenance, but provide referrals to those who do.

What is a Geriatric Care Manager?

Also known as Aging Life Care Professionals, geriatric care managers, usually licensed nurses or social workers who specialize in geriatrics, are a sort of “professional relative” who can help you and your family to identify needs and find ways to meet your needs. These specially trained professionals can help find resources to make your daily life easier. They will work with you to form a long term plan and find the services you need.

What do Geriatric Care Managers do?

  • Discuss difficult topics and complex issues
  • Make home visits and suggest needed services
  • Address emotional concerns
  • Make short- and long-term plans
  • Evaluate in-home care needs
  • Select care personnel
  • Coordinate medical services
  • Evaluate other living arrangements
  • Provide caregiver stress relief
    Depending on the plan developed, they will be able to refer you to other professionals who can help with legal, medical, re-location or other assistance
